The Norwegian Ukom report (2023) on gender care for young people

On 9 March 2023, the Norwegian investigation commission Ukom (Statens undersøkelseskommisjon for helse- og omsorgstjenesten) published its report on patient safety for children and young people with gender incongruence. Among other things, Ukom recommends defining puberty blockers and hormonal and surgical gender-affirming treatment for children and young people as "experimental treatment" (utprøvende behandling) (source). Sweden, Finland and Denmark also describe a more cautious approach.

What Ukom is and what it did

Ukom investigates serious incidents and other serious conditions in Norwegian health care, with the aim of making it safer (source). The investigation into children and young people arose from reports by relatives who contacted Ukom directly; several parties, including authorities, health professionals and patient and relative organisations, were also questioning the safety and organisation of the service (source). Ukom does not set guidelines itself: it recommends that the national guideline of the Directorate of Health (Helsedirektoratet) be revised.

According to Ukom, applying the principles of experimental treatment would provide a framework that ensures information and close follow-up, and would contribute to more knowledge (source).

Co-occurring psychological problems

Ukom cites the national treatment service: 75 percent of those referred have a mental illness, with depression and anxiety the most common diagnoses, and about one in five an autism spectrum disorder, ADHD/ADD or Tourette syndrome; Ukom notes that this matches Swedish figures (source). Norway in the Cass Review

The final report of the Cass Review lists Norway, together with Belgium, Finland and Denmark, among the countries whose guidelines require long-standing gender dysphoria or incongruence for puberty blockers (paragraph 14.15 of the report; source).
